If you go into a terminal program (Terminal, Hyperterminal, Claris Works, etc.) and enter ATI3, what does it display?
When you pick up a voice phone on the line that the computer is on (while offline, as if you were going to dial someone's number), do you hear static or radio noise in addition to the dialtone?

Do you usually hear your modem when you dial out?

Did you hear your modem dial this time?

Did you hear ringing modem tones (handshaking)?

Did the ringing (handshaking) stop and then nothing?

Did the modem tones stop and the modems connect to each other?

Did the ringing (handshaking) not stop? Did it keep going on and on...

Did you get a busy signal?

Did you hear a recording by the phone company?

If yes to the above, what did it say?

Did you see any response from our system?

Did you receive a message about your username or password being invalid?

If yes to the above, did you get disconnected before you logon was complete?
